DEADLINE FOR COMPLETED APPLICATIONS IS March 11, 2008 All completed application packets must be turned in to the Community Awards scholarship coordinator, Mrs. Siverly, 683-4667 x343, by 4:30 pm!

PLEASE NOTE: A few awards have earlier deadlines and must be delivered or mailed directly to the sponsor!

The Yosemite High School Community Award Application will be used for almost all of the Eastern Madera County local awards. Be sure to check the application deadline and special instructions on each individual award cover/information sheet. The application form should be typed; if you don’t have access to a typewriter or computer, complete in black ink as neatly as possible. The general application packet is to be photocopied and stapled to the back of each individual award cover information sheet. Do not put in a binder or envelope! One additional copy of the application packet must be submitted for the YHS file. Staple to YHS file cover sheet and put on top of your pile. It is recommended that you keep a copy of all documents for your records. The general award application packet must include the following: The cover/information sheet for each individual award. Completed general award application. Officially signed current transcript (photocopied). Two (2) letters of recommendation. Only one letter may come from someone in Education. No letter from relatives will be accepted. (Please note that all applications must include at least two letters, but some awards require more than two. Please read your award information sheets carefully.) Special essay questions, additional letters of recommendation, or any other special requirements specific to the individual award. If you are selected, you will receive an invitation to attend: Community Award Night May 7, 2008 7pm in the YHS Theatre This award can only be claimed for ONE YEAR FOLLOWING GRADUATION ALL UNCLAIMED AWARDS ARE VOIDED AFTER JULY 31 OF THAT YEAR.
