| The Yosemite High School Community Award Application will be used for almost all
of the Eastern Madera County local awards. Be sure to check the
application deadline and special instructions on each individual
award cover/information sheet. The application form should be typed; if you don’t have access to a typewriter or computer, complete in black ink as neatly as possible. The general application packet is to be photocopied and stapled to the back of each individual award cover information sheet. Do not put in a binder or envelope! One additional copy of the application packet must be submitted for the YHS file. Staple to YHS file cover sheet and put on top of your pile. It is recommended that you keep a copy of all documents for your records. The general award application packet must include the following:
